The South East Kent Crisis Resolution Home Treatment Team is looking for a Community Psychiatric Nurse to join their team which provides a responsive assessment and home treatment service to those experiencing an acute mental health crisis in the community. You will be expected to work collaboratively with Service Users, Carers and other professionals, offering home treatment interventions as an alternative to hospital admission

You will be prepared to work shifts including nights, bank holidays and weekends and be able to undertake Physical Interventions training in order to support persons waiting for assessment in the designated place of safety.

Main duties of the job

Community Psychiatric Nurse will provide a responsive assessment and home treatment service to those experiencing an acute mental health crisis in the community. Detailed job description and main responsibilities
JOB SUMMARY:

• Managing a case load of service users who have complex acute/crisis needs

COMMUNICATION AND WORKING RELATIONSHIPS:

• To apply a comprehensive range of communication skills with service users, relatives, team
members, and other internal and external stakeholders, adapting communication as necessary.
• To effectively liaise with clients, family, carers, advocates and health and social care
professionals regarding client's assessment, treatment and care to ensure continuity of care and
delivery of a coordinated service as a member of the MDT and implement core decisions with
regard to urgent clinical issues.
• To ensure all communication adheres to Trust policy and Caldicott / data protection principles.

Person specificationExperienceEssential criteria
  • Experience within acute MH services

Desirable criteria
  • Crisis team experience


KnowledgeEssential criteria
  • Knowledge of mental health disorders
