South West Kent Community Mental Health Team is currently looking to recruit a Mental Health Practitioner Duty Worker (Band 6) from one of the following professional backgrounds; Registered Mental Health Nurse, Occupational Therapist or Social Workers to join the community mental health team.

We are currently looking for a Duty Worker (band 6) with post qualifying community or urgent care experience and evidence of continuing professional development. It is desirable for the successful applicant to have had experience of working in a Community based setting.

Main duties of the job

The main purpose of the role is to provide advice to known service users, to provide advice to new service users or other professionals when they are seeking help and to carry out urgent assessments

The Duty Worker is not expected to carry a caseload of clients

We work through an assessment and recovery approach with people accessing secondary level mental health services for severe and enduring mental illness. We provide comprehensive multi-disciplinary services and work closely with long term specialist mental health teams (Early Intervention, Crisis and home treatment team) tertiary service providers, primary care, local statutory and non statutory agencies, service users and carers

You will have a strong sense of initiative, excellent team working ability, be an excellent communicator and be able to demonstrate knowledge of high level nursing interventions & community assessment skills

Qualifications or a willingness to undertake training relevant to the post is expected

Detailed job description and main responsibilities
Carry out and coordinate comprehensive, systematic nursing assessments which take account of relevant mental, physical, social, cultural, psychological, spiritual, and environmental factors, in partnership with service user and others through interaction, observation and measurement.
Work collaboratively with other disciplines, service user and their carers to agree a holistic person centred care plan that addresses the needs identified through assessment.

To deliver and evaluate safe, person centred care in partnership with the service user and their carers to support recovery.

To act as a clinical leader supporting best practice in line with NICE guidance.

To facilitate and review approved Quality Improvement initiatives within the care setting.

To adhere to the NMC registration and revalidation requirements.

To support and uphold the most current NMC Code of Conduct at all times.

To support the implementation of current and future Nursing Strategy.

To promote and to adhere to KMPTs strategies, policies, procedures and guidelines.

To support and behave in a way that upholds KMPT Trust values.

To carry out specific duties relevant to the setting e.g. nurse in charge, medicines clinic management, etc.

To practise safely by being aware of and educating on the correct use, limitations and hazards of common interventions, including nursing activities and treatments.

To report any concerns promptly through the appropriate channels and modify care when necessary to maintain safety, acting to keep patients and others safe.

To provide educational support to optimise health and wellbeing; promoting self-care where possible, helping people to make choices about their needs and involve their carers where appropriate.

To safely manage and administer medications and monitor their effects.

To carry out specific duties relevant to the setting e.g. nurse in charge, medicines clinic management etc.
