Job Description

Main duties of the job



Assist in the planning of duty rota's effectively, based on appropriate skill mix and resources.

To undertake triage of patients and refer appropriately.

To be responsible for assessment of need, planning, implementation and evaluation of appropriate treatment and care.

To undertake screening procedures, e.g. health checks, communicable disease screening, chronic disease management.

To assist the medical team with identification and management of chronic disease.

Medicine Management and administration.

To participate in clinical audit.

To encourage members of the team to develop their clinical skills and expertise.

Use clinical supervision to obtain the necessary support, particularly when dealing with the wide-ranging emotional issued faced by the client group.

Participate in regular management supervision with the Head of Healthcare.

Detailed job description and main responsibilities
The Primary Care Team Leader will assist the Head of Healthcare and senior management to lead and develop a team of healthcare staff and visiting professionals. They will work towards providing the highest standard of care for the prison population.

They will ensure consistent and effective nursing practice as well as operational activity.

The role will include taking a clinical lead in developing a wide variety of initiatives to combat communicable diseases and support chronic disease management.

As a Team Leader, the post holder will assist the Head of Healthcare to ensure that prison policies are adhered to within the Healthcare setting.

  • Registered General Nurse (Adult) with current NMC registration.
  • Significant demonstrable years post registration experience
