Job Description

Main duties of the job

Subject to Recruitment & retention payment of £1452 per annum.

Undertake and oversee the programme of nurse triage, using recognised holistic assessment tools and utilising advanced clinical skills to assess, making all appropriate referrals.

To understand the importance of clear care planning, through assessment, planning, implementation, and evaluation of patient care.

Using a stepped care approach, develop comprehensive care packages for service users chronic and severe and enduring mental health problem seeking specialist advice from other healthcare professionals where necessary

Ensure clinical practice is patient centred and evidence based, in accordance with the NMC Scope of Professional Practice.

Detailed job description and main responsibilities
Undertake assessment of suitability of service users to be transferred or released using recognised tools and protocols within the context of the care plan.

Deliver effective, safe, and high standards of care that it is within clinical competences.

Adhere to all security regulations within the prison.

To promote optimum health with service user group.

To undertake assessments of patients and refer appropriately.

To be responsible for assessment of need, planning, implementation and evaluation of appropriate treatment and care.

To participate in clinical audit.

Ensuring clinicians are competent, purposeful and provide outcome focussed interventions.

To comply with all relevant regulations and policies including clinical policies of GMMH and prison policies.

To recognise the positive effects, and side effects of prescribed drugs and drug interactions and liaise with the appropriate clinicians regarding this.

Be actively engaged in, and co-ordinate the discharge of the patient under the supervision of, and in co-operation with, other members of the multidisciplinary team.

Ensure high standards of documentation are maintained, on all aspects of care, ensuring that records are contemporaneous.

Communicate appropriate information to patients' relatives as agreed by the patient and the clinical team in line with Trust and Prison policies, and Caldicott guidelines.

Manage workload based on flexible principles and prioritise care based on available staff and skill mix.

Person specification

Experience
Essential criteria
  • Experience of working as a Band 5 or Band 6
  • Experience of managing and leading teams, with evidence of managing change and/or difficult situations.
  • Experience in working with complexed service users


Qualifications
Essential criteria
  • Registered Mental health nurse


Knowledge
Essential criteria
  • Knowledge of current NHS agenda and clinical governance relevant to practice


Desirable criteria
  • Worked with women
