Clinical Case Manager

Chase Medical is currently recruiting for a Clinical Case Manager to work as part of a close-knit team, within a service that has an outstanding reputation in the West Lancashire area. The role is a full-time position, on a 3-4-month contract initially but with a view to extend to a permanent contact afterwards.

The successful candidate will receive flexible hours, a highly competitive salary of £40,000 - £46,000 per annum, 5 weeks annual leave, pension and indemnity. To be considered you will need experience in a similar role, experience leading a team, clear and be a precise communicator and have strong organisational skills.

You will be responsible for planning, coordinating and reviewing care for individuals with varying intensity of needs. Liaising with Clinicians and the wider community and urgent care teams. Managing the wards, reviewing patient's clinical observations. supporting and following cases through from opening to closure and coordinating recourses within the wider team.
