Our Adult Community Services is a multidisciplinary team consisting of Community Nurses, Social Workers, Social Care and Support staff. We are looking for a registered Specialist Nurse Practitioner in the field of District Nursing who will take continuing responsibility for the management of a defined caseload within a geographical location. Working alongside a second District Nurse, you will lead and manage a team of nurses to provide high quality patient care.
In addition, you will be involved in the on-going assessment of patient’s needs, utilising the nursing process for planning, implementation and evaluation of care programme. Key will be supporting, guiding, and teaching relatives/carers to give adequate care between visits, and to promote the psychological and physical comfort and rehabilitation of patients.
From a clinical perspective, you will need to develop and maintain skills required to practice at an advanced level and to use extended scope of practice. One of our new initiatives for 2018 is to develop a complex wound clinic, where nurse prescribing will be fully utilised, as well as in the community.
As a District nurse you would also be supporting specialist practice training for District nursing. With this in mind, a practice educator qualification would be desirable. Other skills required would be wound care/ leg ulcer management including Doppler, use of syringe drivers for symptom control, venous sampling, and all types of urinary catheters.
It would be expected that you take a lead role in chronic disease and complex care management including adult safeguarding. Evidence of leadership/management skills would be required as you will share the role of leading and managing a team of community nurses
