Sister / Charge Nurse - Hospital Discharge Team

We are seeking a dedicated and enthusiastic Sister/Charge Nurse to join our client's Discharge Team at their acute hospital site located in the leafy and comfortable northwest suburb of Harrow, Greater London , with good rail and tube connections.
The role is to work in conjunction with and as part of wards based Multi-Professional Teams and provider agencies to develop discharge plans that enable patients who are assessed as medically fit, but have complex care needs to be discharged safely into their homes/ home cares in a timely and safe manner in line with the Delayed Discharges Act 2003
Our client is a leading large provider of UK independent healthcare, working with many of the capital's most experienced consultants and multidisciplinary healthcare - They have 120 beds including ITU, General and Surgical Wards, Spinal and Orthopaedic, Day Surgery, Urgent Care Centre as well as five theatres an Endoscopy Suite and an Ambulatory Care Unit.
Registered Nurse with full NMC registration.
At least four years' post-registration UK-based experience in Acute Surgery or Critical Care.
Current experience in Hospital Discharge processes.
Exceptional communication skills and used to liasing with Multi-Professional Teams.
