Description: Most people living with a life limiting disease would like to stay living in their own home. So this outstanding community palliative care team work to achieve this by forming a vital link between home and the expertise of hospice care.

Person Specification: As part of the Community Clinical Nurse Specialist team you will liaise and work with GPs, District Nurses and all members of the MDT, providing specialist knowledge and expertise in palliative care to patients and their families/carers in their own home, care home or community hospital.

The service is provided over seven days a week 9am to 5pm with the availability of advice out-of-hours. The team accept referrals at any stage of the disease process supporting both the patient/their carers and are a resource to community health care professionals (HCP) assisting with clinical and supportive management, advance care planning and decision making towards the end of life.

You be a registered nurse with a BSc in Oncology / Palliative Care and Evidence of on-going professional development and reflection in palliative and end of life care.
