Altra Care are working with a CCG on the outskirts of London for a Band 8A CHC Manager position. The individual will manage the commissioning side of the Continuing Healthcare pathway, working in partnership with other organisations to ensure best value outcomes with a focus on quality assurance and continuous improvement.
To lead in planning, developing, implementing and monitoring a range of contracts and Service Level Agreements for Continuing Healthcare placements. To support the Clinical Commissioning Group (CCG) agenda to improve standards of care and to work with the CCG’s commissioning strategies
To work in partnership with health and social care to develop and improve Joint Contractual, Quality and Procurement Processes.
To lead on implementing the personalisation agenda within Continuing Healthcare for the CCG.
Specialist in continuing care, working directly on behalf of the CCG to ensure the smooth management of continuing care processes locally.
Representation of NHS commissioners at the relevant Continuing Healthcare panels.
Securing appropriate packages for individuals eligible for Continuing Healthcare that meet quality and value for money standards.
Reviewing and renegotiating where relevant client contracts in line with the Continuing Healthcare client review process.
Facilitating the implementation of personalisation.
The post holder will work with the Children's Continuing Care team to ensure that the transition process from Children's to Adults services is of a high standard and is compliant with the national framework.
Provide expert advice to the CCG regarding procurement of continuing care services.
Responsible for co-ordinating the whole process of commissioning of Continuing Healthcare placements on behalf of the CCG.
Provide effective management of budgets within the areas of responsibility, and provide regular updates to the senior manager in relation to spend against the continuing health care budgets.
To support the service leads across the CCG in developing services for improved patient outcomes.
Provide the relevant support to promote a culture of 'safety first' within the CCG.
In line with the national framework ensure that care reviews are completed at least annually after the first three month review.
To be aware of national and local policy development in relation to CHC and to work with colleagues to implement changes as necessary.
Deputise for Adult Commissioning Manager in their absence, attend meetings on their behalf and ensure the CHC Commissioning Team is appropriately supported and supervised.
To manage the Continuing Healthcare Placements Officer. Offer them supervision and conduct regular 1:1 meetings with them to ensure that they are appropriately supported and perform well.
Work with commissioning and operational managers to develop and implement business plans and in ensuring CHC processes are followed correctly.
Keep accurate and contemporaneous records in keeping with CCG policies.
Work with the commissioning and operational managers in adapting needs of the service from both internal and external influences to identify changes both to processes and personnel.
Assist in the recruitment and retention of staff as required by the changing needs of the service by monitoring and analysing skill mix required.
Ensure benchmarking and audit is carried out effectively and is linked nationally and within
Develop pathways and processes with other organisations as required to ensure best use of
resources and best practice (i.e. Care Brokerage).
Work with other departments within and external to the organisation to contribute to development of service (i.e. working jointly with Health and Social Care Quality Assurance Team).
Ensure local implementation of national policies and strategies complement the CCG’s objectives, vision and strategies.
Attend meetings as a representative of the Commissioning Directorate as appropriate. Developing knowledge of, and interpreting NHS regulations and guidance for local implementation. Communicating this guidance to CCG staff and/or independent contractors as necessary.
Participate in relevant internal and external working groups/projects, services, and initiatives which may be highly complex, sensitive, political, and contain contentious information with the aim of providing information and analytical advice to the teams.
Undertake complex and detailed information analysis of specific projects/reports requiring high levels of concentration
Contribute to the formulation of plans of up to three year and strategic direction within the team on issues, taking a lead for specific workstreams.
