About the Hospital:
My client's has treated over 2,000 patients in their homes. These patients have had a range of high acuity conditions including Chronic Obstructive Pulmonary Disease (COPD), Chest Infection, Pneumonia, Cellulitis, Acute Kidney Injury, Post-operative Surgical and Orthopaedic care, Electrolyte and fluid management pre and post-procedure e.g. Angiography and Gastro-Intestinal investigations.

Accountable to: Director of Nursing & Clinical Services

Job details:
To work closely with the multidisciplinary team and take direction from the Nurse Team Leader in providing and supporting the nursing service.
To provide nursing care for acute medical patients requiring care within their own homes.
To carry out the duties required over nominated shift patterns to ensure the service is delivered over a 24-hour period.
To provide supervision and leadership to the HCA team.

Specific Responsibilities: Patient Care
To demonstrate sound knowledge of clinical practice in the field of acute nursing ensuring that all care is delivered to an agreed standard.
To plan and deliver care for patients and carers appropriate to individual need ensuring that discharge planning commences at time of admission.
To encourage patient independence and empowerment, recognising and respecting their involvement in the planning and delivery of their care.
To provide excellent communication skills enabling patients and their carers to understand their condition and management.
To act as a patient's advocated where necessary.
To provide treatments, including administering prescribed drugs and other interventions.
To actively participate in health promotion opportunities according to individual need.
To evaluate outcomes of interventions.
To ensure that high standards of record keeping are met at all times and that the electronic patient record iNurse and Cross Care contain contemporaneous entries.
To have a flexible approach to the working day in order to meet the needs of the service.

To practice in accordance with NMC professional Code of Conduct and maintain professional registration.
To keep up to date with current developments in nursing practice.
To ensure personal and professional development, maintaining a professional portfolio.
To preserve confidentiality and be aware of the Data Protection Act, Access to Health Records and Consent for Treatment Policies
To attend statutory / mandatory training.

Qualifications/ Skills/ Experience
Registered Nurse
Current experience as an acute care nurse
Evidence of continuing professional development
Sound verbal & written communication skills
Good interpersonal skills, with the ability to establish and maintain effective working relationships
Able to work effectively across professional disciplines
Able to organise and manage own/others workload to meet care requirements
Effective problem solver who meets deadlines
Ability to support & develop others
IT skills
Full current driving license
Car driver
Flexible approach to work
Able to work on own initiative

Post-registration qualification in relevant subject
Practice or community nursing qualification
Experience in guiding and supporting HCAs
Clinical Supervision
Qualified as Nurse Prescriber
Knowledge and understanding of the NHS
Knowledge and experience of clinical audit
European Computer Driving Licence
