Bupa home health care is currently looking for a Nurse Care Coordinator.
This is a senior role with in the community, and you will be responsible for co-ordinating a team of community based staff nurses and Clinical support workers. You will also be responsible for assessing the care required by clients in the community taking into consideration the long-term physical, psychological and social impact a specific diagnosis and treatment can have upon a client’s rehabilitation. In this role you’ll have the ability to recognise the need to adapt education and training to meet individual clients’ needs.
Specific Skills
• RGN qualification essential
• Previous community experience is an advantage
• Previous Adult and Paediatric experience
• Previous management experience or supervisory/mentoring experience desirable
• Experience in managing patients with complex care needs
• Experienced with working with patients on Ventilators and with Tracheotomy’s
• Ability to communicate with client, client’s family and a range of health and care professionals
• Willing to travel to clients homes. Full driving license essential
• flexibility a must
Key Responsibilities
• Mentoring and training staff nurses and clinical support workers in the community
• Working in complex care settings, with ventilators and managing condition such as epilepsy
• You will be responsible for ensuring that you work effectively with your team and customers
• You’ll ensure that you deliver a quality and effective service, empathising with patients and respecting their dignity
• Prioritise requirements of role and ensure that requirements are met each shift
