We’re looking for Nurse Care Coordinators, Drivers essential, who understand the principles of health promotion and the disease process to apply these within practice. You’ll assess the care required by clients and take into consideration the long-term physical, psychological and social impact a specific diagnosis and treatment can have upon a client’s rehabilitation. In this role you’ll have the ability to recognise the need to adapt education and training to meet individual clients’ needs.
Key Responsibilities
• Mentoring and training community nurses and clinical support workers out in the community
• Working in complex care settings, with ventilators and managing condition such as epilepsy
• You will be responsible for ensuring that you work effectively with your team and customers
• You’ll ensure that you deliver a quality and effective service, empathising with patients and respecting their dignity
• Prioritise requirements of role and ensure that requirements are met each shift
Specific Skills
• RGN qualification essential
• ENB998 Desirable qualification
• Previous community experience is an advantage
• Previous paediatric expirence is an advantage
• Previous management experience or supervisory/mentoring experience desirable
• Experience in managing patients with complex care needs
• Expirence with ventilators and Tracheotomy
• Ability to communicate with client, client’s family and a range of health and care professionals
• Must be flexible to travel to clients homes. Full driving license essential
